The Autistic Spectrum Condition Case Management Service consists of two Social Workers from Sirona, Care and Health. The service links into the Complex Health Needs Service and draws on a range of its health care professionals. The service is aimed at adults who have been diagnosed with an Autistic Spectrum Condition (ASC) where their primary needs relate to their ASC. The service also works with young people with ASC during the transition from Children Services (16 years) to Adult Services (18 plus years). The service is committed to the inclusion of people with ASC within mainstream services and the wider community. All people with a diagnosed ASC are entitled to a Community Care Assessment, eligibility for funded support and care management is based on meeting the Fair Access to Care Criteria. The function of the service includes setting up personal budgets, reviewing existing services, identifying and working within adult safeguarding procedures and offering sign posting to other relevant organisations. The service recognises the needs of carers and can carry out separate carer's assessment.
