This template letter can be used to request support following an autism diagnosis. Please copy and paste the draft letter below into a word processer (for example: Word, Pages or Google Docs) and amend the wording before you send it by:
- removing this section of text and the title
- changing the text in [bold] below to match your details and circumstances.

Dear Sir/Madam,
[Your name and date of birth/name and date of birth of your child]
[I/My child] received a diagnosis of autism on [date of diagnosis report/letter].
According to quality standard guidance issued by the National Institute for Health and Care Excellence (NICE QS51), [I/my child] should receive a personalised support plan based on an assessment of individual needs.
I understand that the plan can include information about support groups, help with day-to-day life, [choose one example relevant to your situation from the following: school/college/university/work] and leisure activities, as well as advice about money and benefits and help to prepare for the future.
The NICE guidance says that I should be contacted within six weeks of the diagnosis assessment ending to discuss what support [I/my child] can access in the local area.
[I/my child] should be offered a named keyworker who will help to ensure that the personalised plan is implemented and also reviewed as necessary.
[Scotland only:
Although I am aware that the NICE guidance does not apply to those living in Scotland, I would still like to request a support plan/key worker in accordance with best practice.]
