City Care Partnership provides care to young people over the age of 18 & adults with Autism and/or a learning disability.
We work along-side people, their families, carers, friends & advocates to provide a person-centred package to meet individual’s needs. This include promoting independence, rights & choice, health & well-being whilst ensuring inclusion in their community.
