Knowsley Adult Provider Service - Day Services

Knowsley Adult Provider Service - Day Service offers daytime leisure and life skills activities for autistic people and people with learning and physical disabilities in the community and from bespoke buildings. Using a person-centred approach, the service promotes independence, choice, and control.  It is an inclusive organisation where people are encouraged to reach their full potential.  

Environment
The service operates from activity bases in five localities around Knowsley. The activity bases are in multidisciplinary buildings ranging from leisure and community centres to a bespoke standalone building. Each building is central to their own community and promote a positive environment, to meet the health and wellbeing of people who use them. 

Complex Behaviour and Health Support Needs are met via a bespoke care package, which is implemented by designated staff who work alongside a Knowsley multidisciplinary team. The support plan is constantly reviewed to ensure that individual needs are met.  

Transition
All transitions into the service, begin with a sharing of information before an individual support plan is written. Transitions are a multi-disciplinary and collaborative process with the individual central to decisions and plans made. Transitions are taken at the pace set by the individual and reviewed regularly during the early weeks/months as required.
