ASPIRE will work with young people leaving school who have Asperger syndrome or high-functioning autism, supporting the move from school and children’s services to post school options. We will work with each young person using a person-centred planning approach. Our objective is to promote independence and social inclusion. We will consult parents or carers and work jointly with schools, Health & Social Care, Children & Families and the Local Area Co-ordination Team. We have close links with colleagues in health, careers, colleges and the voluntary sector and aim to develop integrated working to plan and manage transitions as smoothly as possible. If a young person has Asperger syndrome or high-functioning autism and requires some help with planning for transition, a referral can be made to ASPIRE from age 14. ASPIRE will: assess need, working with the young person, their family and those who know them best; help the individual and their family to explore options and make a plan for the future; provide opportunity for a social support group; check that the young person is receiving the benefits they are entitled to; and support individuals as they adjust to new environments and new experiences.
