The Disability Register's aim is to enable the effective planning and coordination of services for disabled children/young people in Conwy and Denbighshire. The coordinator: maintains and develops the Disability Database within the Child Health System working in partnership with all the relevant agencies and users of services; Provides information including newsletters, service directories and statistical reports to help inform, improve and shape service developments for disabled children in Conwy and Denbighshire; Enables parents, carers and children to have a voice in service delivery via a Family Partnership Group.
