Green Lane supported living
Morden
Main information
Type of service: Supported living schemes
Green Lane is a supported living service for adults aged 18-35 with learning disabilities, autism spectrum conditions and associated mental health problems. The aim of the service is to reduce dependence on staff and provide the necessary skills for the individuals supported to thrive independently both inside and outside of the home, so that they might then be able to move on to greater independence in the future. Our people are supported by developing person-centred plans in order to develop independent living skills through in-depth skills teaching.
Individuals will receive Total Communication support with individualised plans tailored towards specific needs and focused on outcomes. Staff are available 24 hours a day and provide intensive support and a consistent approach with a focus on encouraging social, communication and life skills development.
