Manor Farm is our new Transition Service located in Cambridgeshire. This service is for young people (aged 16-25 years) with learning disabilities and/or autism. Manor Farm has been created as a 'move on' service for school leavers and those looking to move out of the family home but do not yet have the skills required to live independently. We have designed the service to give young people: An opportunity to live within a small residential home with their own bedroom and en-suite facilities; Person-centred support based on identified needs with clear outcome objectives; Training and support to improve their daily living, self-care and communication skills; and The goal of moving onto Supported or Independent Living services as their skills develop. We will provide reassurance to each young person's family, care team and funders through: quality care: A progressive care pathway for even the most complex people; Use of outcome tools to evidence progress, including 'Life' and 'The Spectrum Stars' which shows tangible and provable results; Therapeutic Team support when required, to assist skills development, and minimise the risks of placement failure; and Robust and meaningful review programmes; regular reviews with Care Programme Approaches (CPAs); A trusted partnership: Environments and service provision as evidenced by the Care Quality Commission; Value with a transparent sliding scale of fees linked to progress and outcomes; and Relationship with Brookdale in seeking appropriate 'move on' services. This includes a commitment to meaningful transition to new providers. Accomplish will offer follow-up support, advice and training if required for the new provider. We provide care pathways enabling individuals living with autism, mental health and learning disabilities a variety of routes to independent living. From our therapeutic campus to residential care and supported living we specialise in providing the right type of care at the right time in a person's life. We also provide respite support services, to allow short breaks for full time carers or a 'between placement' service.
