Pathway House is a residential service linked to Milton Park Therapeutic Campus that enables people we support to avoid inappropriate admissions to an inpatient setting, or to be discharged safely through a well-planned transition from specialist hospital care. Pathway House provides a supportive, person-centred environment which enables each person to build their confidence and gain real choice and control over their lives, whilst working actively towards more independent living.Pathway House is designed to offer a step up/step down residential service that prevents unnecessary hospital admissions. The service outcomes include: Successful transitions from hospital (Step-down); Avoided inappropriate hospital admissions (Step-Up); Reduction of re-admissions to hospital; Progression from this service to more independent living settings; and People we support and their families or carers are satisfied with the service. Pathway House's proximity to Milton Park ensures that there is access to a multi-disciplinary, clinical team that includes Psychiatrists, Psychologists, Speech and Language Therapists and Occupational Therapists who will provide support if a person we support experiences a time of crisis. Specialist training enables the Pathway House team to gain a clear understanding of the person we support. This includes the ability to support people to improve their living and social awareness skills which are further developed through accessing the community. Our approach includes supporting people to access college, travel training and leisure opportunities.
