Reach Out is a web-based resource designed for all professionals in the social care sector to support the move towards personalised day opportunities. Developed by Paradigm in partnership with eight local authorities, Reach Out uses the Eight Reach Out Community Outcomes that accurately reflect what people want in their lives to encourage and enable them to play an active part in their local communities. Reach Out then guides professionals to ensure individuals achieve these outcomes through a specifically designed change management process. The website is free to access and the outcomes are FREE to download and use. The Members Only section of the site requires subscriptions between £600 to £1950 depending on the Local Authority/Trust/Independent Provider and how many people are supported by them. This gives you access to a complete range of resources to facilitate: A process for real change based on the wishes of the people supported; An online community forum to share ideas, resources, good practice; The collection of information and aggregates this to shape the vision, add content to plans for change, and give details for action; The ability for commissioners and those responsible for personalisation to evidence whether or not services deliver the Eight Reach Out Community Outcomes; The monitoring of individual services in terms of quality and price; A process for costing and pricing services to enable people with personal budgets to purchase directly; The benchmarking of day opportunities, locally, regionally and nationally. It has already been purchased by 22 local authorities.
