Type of service: Outreach - education

The inclusion team offers advice and support to children and young people, schools and families. It aims to support the development of inclusive practices in schools and settings. Professionals from a number of different disciplines work within the service: Educational psychology, Learning support, Sensory support, Family support for children with disabilities, The statutory assessment team. Referrals can be made to the inclusion team using the FSM01 form. Referrals can be taken from schools for all parts of the team other than the family support for children with disabilities - families can make referrals directly for this.
